if you look at his pictures chronologically, he clearly gets less feminine in body/face shape as time goes on, implying the use of hormones, which he said he was on at one point.
he also confirmed for being epileptic, which is closely tied in with intersex defects. so it might be possible he has a non-functional/superfluous penis which that pic was meant to compensate for since he did identify as a trap
